How DVA Care Works
Getting started with DVA health services is straightforward. Here is how we can begin supporting you or your loved one.

Contact us by phone or through our enquiry form. Whether you are a veteran, a family member, or a referring health professional, our team will guide you through the process with care.
Care Assessment
We work with you and your healthcare team to understand your specific needs. This includes reviewing your DVA entitlements and developing a personalised care plan that fits your life.
Begin Services
Once approved, we match you with a care professional who is right for you and begin delivering services. We maintain open communication with you, your family, and your referring practitioners throughout.
DVA Service Eligibility
DVA Health Cards (Gold and White) are accepted as payment upon GP referral. If you hold a valid DVA health card, you can access clinically required nursing and personal care services in your home.
DVA Gold Card
As a Gold Card holder, you can access our full range of community nursing, personal care, and home support services for any health condition upon GP referral. Simply ask your GP to send a referral straight to us.
DVA White Card
As a White Card holder, you are eligible for services related to your accepted service-related conditions. Your GP can refer you directly to Commcare, and we will coordinate with DVA to confirm your entitlements before services begin.
Community Nursing
The DVA Community Nursing program provides clinically required nursing and personal care in your home, including wound care, medication support, hygiene assistance, and palliative care. Ask your GP to send a referral directly to Commcare and we will take it from there.
Veterans' Home Care
The Veterans' Home Care program provides practical support so you can continue living independently in the home you know and love. Call the VHC Assessment Agency on 1300 555 450 for an assessment, then request Commcare as your preferred provider.
Please Note
Community nursing services are available for veterans living at home. Those residing in Residential Aged Care Facilities receive nursing care through their facility. All services require a recommendation from an appropriate healthcare professional.
Making a Referral
Community nursing referrals can be submitted directly to Commcare. We aim to begin services promptly so your patient receives the care they need without delay.
Accepted Referral Sources
A written referral for DVA community nursing services can be made by any of the following:
- General Practitioner (GP)
- Hospital doctor or specialist
- Hospital discharge planner
- Nurse practitioner (community nursing)
When is a Referral Required?
A written referral from a qualified healthcare professional is required when a veteran is:
- Starting community nursing services for the first time
- Returning to services after a gap of more than 28 days
- Renewing at the end of each 12-month care period
